The programs offered by SNAP are intended to assist a wide variety of clients, including 496 children and students who fall into any of the 13 special education qualifying categories.
We support families in navigating the complex special education system, making sure that individualized education plans (IEPs) are customized, and speaking up for the needs of the students. Our advocates provide assistance in creating and evaluating IEPs, keeping track of student progress, and participating in staff meetings at schools. We also direct families to local resources, educate them about their rights under special education regulations, and offer emotional support. Our all-inclusive transition planning services, which address topics including guardianship, enrolling in benefits, housing aid, and vocational rehabilitation, get students ready for life beyond high school.

An initial consultation with the kid, family, and Special Education Advocate kicks off the Paragon Home Health Care SNAP Process. The first meeting will provide the groundwork for the advocacy process by giving parents and students a chance to voice their concerns, clarify expectations, and make goals. In addition to outside service provider notes and evaluations, education records, assessments, and IEP/496 paperwork will all be examined by a special education advocate. When a patient’s condition is critical, a Nurse Client Advocate may collaborate with the team.
